PROGRAM PRE-REQUISITES

  • Credit and reference checks may be required.
  • Contractor should be established for a minimum of two years.
  • The Last 3 Year-end CPA prepared business financial statements - complete.
  • Work-On-Hand Schedule
  • Bank Reference Letter (sample included in application)
  • Recent personal financial statement of each of the owner(s)
  • Certificate of Insurance

APPLICATION INSTRUCTIONS AND REQUIREMENTS

  1. Download the Performance Packet - Performance Packet (PDF 795kb.)
  2. Fill out the Contractors Profile.
  3. Fill out and Return the Work-On-Hand Schedule (located in packet)
  4. Obtain a Letter from your bank using the supplied example from packet
  5. Have any and all owners complete attached Personal Financial Statement (included in packet)
  6. Fill out appropriate Bond Request Form (attached in packet)
  7. Contract/Agreement - (A copy of the entire agreement is necessary for us to determine project acceptability.
    For bid bonds, please include bid specifications provided by the Obligee. Please include all required bond forms
    for review and acceptance.)
  8. Return the Contract, Performance Packet application along with all other required documents
    to the following address:
